My Aged Care is a national online and phone service to help you find out about aged care services, and what services may be available to help you.
It is a one-stop portal that provides information, means and referrals to various services (including community transport) to enable healthy and active lives for non-indigenous Australians aged 65 and over, and indigenous Australians over the age of 50.
A onetime registration with My Aged Care will save you the trouble of repeating the process of registering your details with each aged care provider thereby saving you time and effort.
My aged care gives you more choice, more control and easier access to a wide range of aged care services.

LINK and My Aged Care
LINK Community Transport is a registered My Aged Care provider, offering safe, reliable and affordable door to door transport to a range of social, recreational, health and family commitments.
My Aged Care refers people who indicate they have transport needs to transport providers like LINK. Once a referral is received, our intake team will be in contact with you to assess your particular needs and register you as a client so you can start traveling with LINK.
If we are your preferred provider, please ensure that you request for LINK to be your transport provider when liaising with My Aged Care.
